Nodes with no `access_tags` key in metadata are never filtered by `visible_to` — they are treated as unconditionally public.

Summary

Any node that lacks access tags in its metadata will always show up in query results regardless of who is asking. This is the default visibility behavior — if you forget to tag something, it becomes visible to everyone rather than hidden from everyone.
